We are pleased to announce that open enrollment Life Insurance is available for a limited time only. Voluntary group coverage is available for you and your family to purchase Life insurance, Accidental Death and Dismemberment Insurance at group rates. Coverage is Guaranteed up to $150,000 and there are NO MEDICAL QUESTIONS OR PSYCHICAL REQUIRED.
If you are currently enrolled, your benefits will continue as is at your current rate unless you choose a different option.
Member Voluntary Life Insurance: Up to $150,000 ($25K increments)
- Pays advance of up to 75% of benefit to $150,000 if diagnosed with a Terminal Illness. (see policy provisions)
- Convenient Payroll Deduction
- Convertible to Individual Whole Life Policy when you retire or separate from service without evidence of insurability.
- Premium Waived if you become totally disabled
Member Accidental Death & Dismemberment Benefit: Up to $150,000 ($25K increments)
- Pays additional schedule of benefits if as a result of an accident you lose a hand, foot, sight, hearing, or suffer paralysis
Spouse Voluntary Life Insurance: Flat $20,000
Child Life Insurance: Flat $5,000
